Annual Records Solicitation Form Announcement

Your trusted advisors at Lewis & Knopf, CPA, P.C. would like to make you aware of a bogus Annual Records Solicitation Form being mailed to Michigan businesses. The fake form looks official and requests businesses to send $125, along with the completed form, to 5859 West Saginaw Highway #343, Lansing, Michigan. Please be aware the real form will require a $25 submission fee to be mailed, along with the form, to P.O. Box 30054, Lansing, Michigan, or to be completed online.

The Michigan Department of Licensing and Regulatory Affairs will send all corporations and limited liability companies information on filing their 2017 annual report or annual statement three months prior the form’s due date. All for-profit and professional service corporations are required to file their annual report by May 15, 2017. Limited liability companies (LLC) and professional limited liability companies (PLLC) are required to file by February 15, 2017, and non-profit company filings are due by October 1, 2017.

The report can be filed online at www.michigan.gov/fileonline beginning 90 days prior to the due date or a paper copy can be mailed to the Michigan Department of Licensing and Regulatory Affairs Corporations, Securities, and Commercial Licensing Bureau. All reports not filed by their due date are subject to a $10 per month late penalty fee (not to exceed $50).
